    Santa Clara County and Contra Costa County require a Proposed Order (Cover Sheet) Judicial Counsel Form EFS-020 when you are electronically submitting a [Proposed] Order, or a Stipulation and [Proposed] Order.

    Once you efile the Proposed Order (Cover Sheet) attaching a pdf version of the [Proposed] Order, you then must email the pdf of the Proposed Order (Cover Sheet) (EFS-020) AND a word version of the [Proposed] Order to the Department Judge.

    What other counties require this form and procedure?

    I just filed a Petition in a PROBATE matter in Sonoma County and efiled the proposed Order with a cover sheet and emailed it in Word format to the probate examiner. I would guess that the Civil Department in Sonoma would require the same.
